Applications can be obtained at our office: 56 S 65th St. Suite 3 Belleville, IL 62223. They’re $40 for the first adult applying, $25 for each additional occupant over the age of 18. We will need a copy of a driver’s license or state ID, social security card, and 2 months’ worth of paystubs to accompany each application.
Specific breeds of pets are welcome with a $250 nonrefundable pet fee per pet.
